Thank you for your interest in a Ride Along w/ our agency! We are located in unincorporated King County, Washington and provide fire and EMS services to areas of Auburn, Kent as well as The City of Black Diamond & Muckleshoot Indian Tribe.
Once we have reviewed your requested dates, we will do our best to accommodate your request & will send a confirmation email, including the selected date, station & info.
Requests are to be made a minimum of 2-days/ 48-hours prior to a requested ride along.
Ride along timing will typically be between the hours of 8 AM and 6 PM but may be shorter for various reasons (training events, incidents, stations availability, etc.).
No one may attend a ride along without confirmation back from the fire department, Fire Chief or their designee, that the request has been approved.
Confirmation of an approved or denied request will be sent via email. Approved requests will include the department selected date, time frame and station location.
This form is a request, and may be denied without cause or reason.
Emergency medical and/or other related incidents can expose emergency workers and any other personnel to potential contamination by infectious diseases and/or bloodborne pathogens. This includes but is not limited to; HIV, AIDS, Hepatitis, TB, and many other related life threatening diseases. Many of these infectious diseases have the potential to cause illness and death. At no time will a civilian rider be allowed at or placed in a situation that contributes to exposure of infectious disease or bloodborne pathogens. Fire Department personnel are specially trained and provided with protective equipment and inoculations to protect from Bio Hazards
In consideration of receiving permission from Mountain View Fire and Rescue, to ride as an observer in Emergency Vehicles, owned and operated by the Fire District during Emergency and Routine responses and business, the undersigned hereby releases Mountain View Fire and Rescue, its Commissioners, agents, officers, and employees of and from any and all liability, claims, demands, actions, and causes of action arising out of or related to any loss, damage or injury, including death, that may be sustained by the undersigned, or any property of the undersigned while in or on the premise of the District or in any emergency vehicles owned or operated by the District. The undersigned is aware of the risks and hazards inherent in riding in emergency vehicles while responding to or returning from an emergency response call. The undersigned hereby elects voluntarily to enter the District premises and occupy District vehicles and assumes all risks that may be sustained during such activity. This release shall be binding upon the heirs and personal representatives of the undersigned.
By checking the "I agree" box below, you agree and acknowledge that 1) your application will not be signed in the sense of a traditional paper document, 2) by signing in this alternate manner, you authorize your electronic signature to be valid and binding upon you to the same force and effect as a handwritten signature, 3) you may still be required to provide a traditional signature at a later date and 4) that you understand and will abide by all information as outlined in this form and in no way hold the department liable for any exposure, accident or injury.
By checking the "I confirm" that I will be 18+ years of age by the date(s) of the requested ride along(s). If I (the rider) will not be a minimum of 18+ years of age by the date of the requested ride along(s), than my parent or guardian is to sign below in my place.
(if signed by parent) I, the parent or legal guardian of the above mentioned minor, am signing on behalf of the minor rider (signature above) and give my full permission for them to attend a ride along(s) with Mountain View Fire and Rescue, understanding and confirming to all rules, waivers/ releases, risk and information as outlined in this form. I will in no way hold Mountain View Fire and Rescue liable for any accident, injury, trauma or potential exposure.
